Opt-Out forms are not all the same. Be careful that your school district DOES NOT block distribution of your child's information to colleges or any other post-high school situation other than the military. In the NYC example, you have three different options. If your local high school doesn't give out opt-out forms, CREATE ONE ! Using your schools letterhead, paste it on the body of the a form that you like and hand them out where ever students congregate. They should be returned publicly or by certified letter... It's The Law!
